What is LifeStrive®?
LifeStrive’s health promotion system enables you to discover your health risks, track your health trend over time and learn what improvements you, along with your physician’s guidance, can make to your lifestyle to help prevent chronic illness! The LifeStrive® health package is available when you assign the wellness benefit that is built into your Allstate Workplace Division Cancer coverage, under the high or premium option.

What Lab Tests are included in my LifeStrive® Comprehensive Wellness Panel?
LifeStrive’s comprehensive wellness panel is a prepaid 50-point biometric test which includes:
- Lipid panel (cholesterol, HDL, LDL, risk ratio, triglycerides)
- Complete Blood Count (CBC) (WBC, RBC, Platelets, Hct, Hgb)
- Electrolytes
- Thyroid Panel w/TSH
- Liver Enzyme Panel
- Kidney Panel
- Glucose (Diabetes)
- Mineral Profile
Does my agency, university or community college have access to the information in my LifeStrive portal?
No, your privacy is protected. All health information is completely confidential in compliance with the Health Insurance Portability and Accountability Act (HIPAA). The only information an employer can receive is identity-protected summary data to help them understand the health needs of the overall employee population.
How do my dependants and I access the LifeStrive health package?
The LifeStrive® wellness screening is available when you assign the wellness benefit that is built into your Allstate Workplace Division High or Premium Cancer coverage. See the LifeStrive® Overview to assign your wellness benefit. You can cancel your LifeStrive wellness benefit assignment at anytime, but completing and submitting the revocation form.
