First
of all, do you have the Adobe Acrobat Reader installed on your system?
If not, you will have to download and install it before you can even
see the form.
The Acrobat Reader is free; go to the Adobe
web site and follow the instructions provided. The Reader
is a fairly small file and will not take long to download. Once you have
downloaded the file, double-click on it and follow the instructions on
the screen to install it on your computer. You may have to restart your
computer for the Reader to work properly.
Once
you have installed the Acrobat Reader, click on the "Fall 2008 Registration
Form" link on the right to load a registration form on your screen.
After the page loads in Adobe Acrobat, you have a choice: you can print
the form out to fill it in by hand, or you can type in your answers and
print it using your web browser's Print command (usually on the File menu).
To
type in the form while it's on your screen, follow these directions:
-
Click in a field that requires information; a blinking cursor ("|")
will appear and you may begin typing.
-
To move to the next blank, press your TAB key (next to the "Q" on
your keyboard) or click in the next blank with your mouse. Continue
to move through the form until you have completed all the blanks that
apply to you and your training choices.
-
Once completed, you may print the form with your local printer. THE
FILLED OUT FORM CANNOT BE SAVED TO YOUR SYSTEM - YOU CAN ONLY PRINT THE
FORM. If you exit the reader for any reason, your data will be lost.
It is recommended that you minimize the form, and stay online if you
need to do research for certain information.
Remember, you do not have to fill the form out online - you can print
the blank form, and then type the information with a manual typewriter
or handwrite your information.
If you want to save the form to your computer to print later, follow these
instructions:
